The full Diagnostic Decoding Surveys kit includes the following:
- User’s Guide, which includes detailed information about administering, scoring, and interpreting the Surveys, along with the blackline masters of Recording Forms used to score the Surveys.
- Student Materials book, which contains the pages of words that students read for Forms A–G of both the Beginning Decoding Surveys and the Advanced Decoding Surveys. Inside the front cover is a pocket with the summary directions for administering the Surveys.
- 20 sets of Recording Form Booklets for Forms A–G of the Beginning DS and Advanced DS.

User’s Guide
The User’s Guide is intended to help examiners as they administer, score, and interpret the Diagnostic Decoding Surveys. It is written with three major purposes. First, it explains the Beginning Decoding Survey and the Advanced Decoding Survey to first-time users. Second, it answers questions that new and experienced Survey users may have. Third, it gives many scoring examples to illustrate the various mistakes that students may make on the Surveys.
